Housing difficulties can impact health, and while we understand the challenges, our practice follows local guidance regarding medical reports for housing applications.

Requests for Medical Evidence from Sheffield City Council (SCC)

SCC has agreed that patients do not need to obtain medical evidence from their GP. If required, SCC will contact us directly with written patient consent. As this service is outside NHS obligations, GPs may charge a fee, payable by SCC upon submission of an invoice.

Requests for Professional References for Rehousing

GPs are no longer required to provide professional references for rehousing applications. The absence of a GP reference will not negatively impact applications.

Requests from Housing Associations

Medical reports for housing from Housing Associations are not part of NHS services. Our practice does not routinely provide these reports. If a GP agrees to do so, a fee will be charged, payable by the Housing Association or patient.

Alternative Sources of Support

  • SCC may contact the GP directly if medical evidence is required.
  • Supporting documents from specialists, social workers, or occupational therapists may be helpful.
  • Self-reported health impact statements can be submitted to housing authorities.
  • Patients can request a referral to the Heeley Trust Link Workers via Reception for additional support.
